Orders may be placed in a number of

FAX, MAIL or TOLL-FREE PHONE.

by using the
Add to Shopping Cart Buttons next to
each item listed on this Web site and making payment by credit card using VISA,
MasterCard or American Express. You can view the contents of your shopping
cart at any time by clicking on the View Shopping Cart Button on the home
page. All credit card information being sent and received over
the Internet is at 128 bit encryption (similar to the level of encryption
used by customers with online banking accounts). THIS
CAN ONLY BE USED FOR ORDERS TO BE SHIPPED WITHIN CANADA AND BILLED IN

form (you will need to use Adobe Acrobat Reader -- if you don't already
have it you can download it free from http://www.adobe.com) and send it by fax to Productive Publications at (416) 322-7434.
Payment should be by VISA, MasterCard or American Express. You can also use
your own purchase order form or memo or letter but please remember to include your
mailing address, card number, expiry date and name as it appears on the
card.

Canada is extra and is calculated on check-out at the rate of $9.95 for

PROTECTION
Protecting your privacy and your personal data is an important aspect of the way
we create, organise and implement our activities on-line and off-line. Our Web

Web server for online payments with a secure (using 128 bit encryption) download
from time to time of such data for credit card authorization and order
processing/shipping purposes only.
PURPOSE
SPECIFICATION AND DATA COLLECTION
You can access our Web sites' home pages and browse our sites without disclosing
personal data. Our Web sites log information about you that is automatically
recognized by the Web server, such as your IP address and domain name.
Cookies are used to hold session information and to facilitate the shopping cart and ordering

collect the personal data that you volunteer on registration for our electronic
newsletter, to receive news releases, to place an order, or for surveys, or on
other forms, or by e-mail.

our employees and processors who have access to personal data and are associated
with the processing of that data are obliged to respect the confidentiality of

VISITOR
CHOICE
You
may choose to receive news releases, promotional or marketing information from
Productive Publications by selecting the appropriate option at the point where
our Web sites currently (or in the future) collect personal data, or by sending
us an e-mail.
If
you do not wish to continue receiving news releases, promotional or marketing
information, please send us an e-mail indicating that you want to be removed
from our e-mail databases and please include the e-mail address(es) that you
wish to have removed. We will send you an e-mail requesting confirmation of your
desire to be removed (in order to prevent unauthorized removal) and within a
week of the receipt of this confirmation the e-mail address(es) in question will
be removed from our databases.
INDIVIDUAL PARTICIPATION/ACCESS
You
can ask us, by sending us an e-mail whether we are keeping personal data about
you. You can ask for a copy of the personal data that we keep about you. We will
endeavour to reply to your request within a week; any such reply will be at no
charge for you. You can request us to erase, rectify, complete and amend the
data that we hold about you.
Before sending you any personal data, you will be asked to provide evidence of your
identity. Should you not provide satisfactory evidence of your identity, we
would reserve the right to refuse to provide you with the personal data.
SECURITY
We have implemented technology and security policies, rules and measures to
protect the personal data that we have under our control from unauthorized
access, improper use, alteration, unlawful or accidental destruction and
accidental loss. We will continue to enhance our security procedures as new
technology becomes available.
